INDEPENDENT CONTRACTOR AGREEMENT

 

 

            This Independent Contractor Agreement (“Agreement”), consisting of five (5) numbered pages, is entered into on the first date signed below, by and between ______ doing business at ______  (“Contractor”) and the Company listed on the signature page of this agreement (“Company”).  Contractor and Company are hereinafter collectively referred to as the “Parties.”

 

RECITALS

 

 

  1. Contractor is in the business of ______  (“Contracted Services”).

 

  1. Contractor uses his/her own tools and resources to perform the Contracted Services.

 

  1. Company desires to engage the Contractor to assist the Company in the Contracted Services.

 

  1. Contractor will provide the Contracted Services at an hourly fee, and use its Sources to provide supplies and furnishing for the Company’s home.

 

NOW, THEREFORE, in exchange for valuable consideration, the Parties hereby agree as follows:

 

AGREEMENT

 

1. Duties

 

Contractor shall perform the Contracted Services at the hourly prices set forth in Exhibit A.  Contractor shall perform the Contracted Services in a diligent and professional fashion.

 

2. Payment

 

 

Company will make payment to the Contractor on a monthly basis for hourly services invoiced in accordance with Exhibit A.

 

3. Permits and Taxes

 

Contractor shall obtain any and all necessary licenses and permits for the Contracted Services, whether from local, state or federal municipalities or governments, or similar entities, and shall pay all applicable fees, costs and taxes.  Contractor assumes full and complete responsibility for fines, and other consequences or liabilities arising from failure to obtain proper permits or licenses or to pay taxes.
