This is the basic USPTO form for filing and recording an assignment.  In order to do this, however, you also need the actual assignment document.  On Free Legal Aid, you can find one assignment for an intent to use trademark and another for use-based trademarks. Once you have all the proper documents, get them executed and then file with the USPTO with the attached recordation form.
